Mesa Sports/Athletic Boosters Donation Form

PFO has partnered with the Mesa Union Athletics Program to fund its activities.  In order to maintain and keep or sports programs going, we need your help!  PFO pays for team registration fees, referee fees, uniforms and equipment.

 

To help ensure our coaches and teams have the resources they need, we are asking for a $30 donation per student athlete for each sport he/she is playing.

 

We are also looking for Athletics Boosters.  Boosters will be full voting members of PFO and help provide feedback and direction on the use of athletics/sports funds.
